1. As soon as you move to Cordova, call Native Village of Eyak main office. (907) 424-7738 and speak to the enrollment department.
2. Set up an appointment to fill out Eyak’s enrollment application. Bring your CIB to the appointment.
3. If you do not have a CIB you need to apply for one. Eyak’s enrollment department can help you with this. You will need an original or certified copy of your birth certificate and your social security card. Eyak can assist you with obtaining these documents if needed.
